Consent with personal data processing
I give consent with collection, storage and processing of personal data in this form by the Controller Turistické informační centrum Frýdek-Místek, p. o., náměstí Svobody 6, 738 01 Frýdek-Místek, Czech Republic, ID No.: 66933901 (or its employees) for the below-given purposes. I understand that the provision of data is voluntary, that I can remove my consent any time and I declare that I filled-in all the information truthfully. I give the consent for an indefinite period of time or until revoked. I also understand my rights as per Regulation of the European Parliament and Council (EU) 2016/679 from 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), hereinafter only “GDPR”, especially the right of access to personal data as per Sec. 15 GDPR, correction of personal data as per Sec. 16 GDPR, right for deletion of personal data as per Sec. 17, or limitation of processing as per Sec. 18 GDPR, and also the right to portability of data as per Sec. 20 GDPR and the right to objections against processing as per Sec. 21 GDPR.
The Controller does not engage in any automatic individual decision-making including profiling within the meaning of Article 22 of the GDPR.
I understand I have the right to file a complaint at the Office for Personal Data Protection if I believe my privacy has been compromised.
Purpose of processing of personal data
In compliance with GDPR, all the data in the form shall also be collected and processed for the purposes of sending commercial messages to the data subject by electronic means according to Sec. 7 of the Act No. 480/2004 Coll., on certain information society services, till the data subject directly and effectively sends the information to the Controller that he does not wish that commercial information would be sent by the Controller any more.
The Administrator hereby declares that it will collect personal data in the extent necessary for fulfilment of the defined purpose and will process the information solely in accordance with the purpose for which they have been collected. The controller’s employees or other natural persons who process the personal data under a contract with the controller are required to maintain confidentiality of the personal data even after the termination of their employment or work. Personal data will not be provided to third parties or into third countries.
